When we get together to open
your accounts, we will ask to see the following documentation:
If you are operating as a
Sole Proprietor, please provide:

a current state issued
Driver’s License, Military Identification Card, or state issued
Identification Card,

the Social Security Number
or Tax Identification Number used in the business,

and your Assumed Name
Certificate.
If you are operating as a
Professional Association, please provide:

a current state issued
Driver’s License, Military Identification Card, or state issued
Identification Card,

and the Social Security
Number or Tax Identification Number used in the business.
If you are operating as a Partnership, please
provide:
If your are operating as a Corporation, please
provide:

the Tax Identification Number used in the business,

the Articles of Incorporation

and an authorizing Corporate Resolution for the
account. (We have the forms if you don’t.)
