When we get together to open your accounts, we will ask to see the following documentation:

If you are operating as a Sole Proprietor, please provide:

  • a current state issued Driver’s License, Military Identification Card, or state issued Identification Card,
  • the Social Security Number or Tax Identification Number used in the business,
  • and your Assumed Name Certificate.

If you are operating as a Professional Association, please provide:

  • a current state issued Driver’s License, Military Identification Card, or state issued Identification Card,
  • and the Social Security Number or Tax Identification Number used in the business.

If you are operating as a Partnership, please provide:

  • the Tax Identification Number used in the business
  • and your Partnership Agreement.

If your are operating as a Corporation, please provide:

  • the Tax Identification Number used in the business,
  • the Articles of Incorporation
  • and an authorizing Corporate Resolution for the account. (We have the forms if you don’t.)
